Description:Visiting Rehab and Nursing Services is looking for
an organized and detail oriented Clinical Intake coordinator to
join our administrative team! The Clinical Intake coordinator is
responsible for conducting reviews of clinical information to
determine eligibility for home health services in compliance with
regulatory standards. This role will include reviewing referrals,
documentation, and coordinating with our intake team to obtain
necessary information to process referrals. This Full Time position
offers a rate of $35/hour, in addition to the Benefits shown below.

Intake Responsibilities (Including but not limited to):Conduct
initial patient assessments to determine eligibility for home
health services in compliance with regulatory standards.Evaluate
and process referrals from healthcare providers, ensuring all
necessary documentation is complete and accurate.Maintain
up-to-date knowledge of home health regulations, including PDGM
(Patient-Driven Groupings Model) and Medicare
guidelines.Collaborate with clinical staff and interdisciplinary
teams to coordinate patient care and develop individualized care
plans.Ensure timely and accurate data entry into electronic health
record systems.Monitor and track intake processes to optimize
efficiency and compliance with agency policies.Assist in quality
assurance and performance improvement initiatives related to intake
procedures.Determine medical necessity requirements for home health
services, ensuring compliance with federal and state
regulations.Track hospitalizations and transitions of care to
ensure timely follow-up and appropriate service delivery as needed
in collaboration with hospitalizations role and nursing clinical
intakeObtain intake information from referral sources as needed to
facilitate a smooth transition into home health services as needed

Requirements: Licensed Practical Nurse or Registered Nurse with the
following skill set:Excellent attention to detail and
organizational abilities.Proficiency with electronic documentation
systems.Effective communication skills, both verbal and
written.Ability to work collaboratively in a team-oriented
environment.In-depth knowledge of home health regulations, PDGM,
and eligibility/referral requirements.Strong communication and
interpersonal skills, with the ability to interact effectively with
patients, families, and healthcare professionals.Proficient in
electronic health records (EHR) and Microsoft Office Suite.At
minimum, 1 year experience in the home health setting.Contact us
