Job Summary:
The Per Diem Social Worker - Case Manager is responsible for facilitating care along a continuum, effectively coordinating resources to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination.
Responsibilities:
The Per Diem Social Worker - Case Manager will be responsible for:

• Conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
• Assessing patients for transition needs, identifying and assessing patients at risk for readmission.
• Integrating national standards for case management scope of services, including transition management, care coordination, compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
• Providing education to physicians, patients, families, and caregivers.
• Leading a population of patients by service line and/or leading the team by being a resource to Tenet performance standards.
  Requirements:
  The ideal candidate will possess a Master's of Social Work degree, with preferred experience in acute hospital settings. A current license, certification, or registration to practice as a social worker in the state of practice or policy is required. Active licensure for state(s) covered is also required. Accredited Case Manager (ACM) certification is preferred.
